Insurance
I am a provider in some of the plans of
the following network, please check out whether I am a network provider of your
insurance plan with your insurance company, and check out your deductible and
co-pay.
If I am in your insurance
provider’s network, you will pay a lower rate of deductible and copay, if I am
not a provider in your network, you will pay higher rate for copay, or higher
ceiling of deductible. I will file the claims for you either way. Ultimately, you
are responsible for the payment.
